$36000/Year Clearbrook is a non-profit, social services agency specializing in providing innovative opportunities, tools, and services to individuals with developmental disabilities.
The QIDP is a member of the interdisciplinary team providing holistic programming for individuals with intellectual/developmental disabilities.
This position is responsible for coordinating, planning, and executing programming for an assigned caseload.
A QIDP must have:
Bachelor's degree in social services, human services, or related field At least 1 year of experience working directly with individuals with intellectual / developmental disabilities Case management preferred Valid IL driver's license Responsibilities include:
Actively participate in program management and general staff meetings Conduct team meetings on a regular basis Ensure that safety standards and when applicable, safety devices are appropriately utilized in the program areas at all time.
Maintain confidentiality with all clients and their families Maintain a professional image/role as a representative of the agency Participate as a member of an interdisciplinary team in developing an individual program plan for each assigned client Develop appropriate curriculum for implementation of client programming objectives and ensure the curriculum is being followed Act as QIDP for community clients, coordinating/attendance IDTs, and intake/discharge staffing Full Time Benefits Package:
